
As a courtesy, Health Billing Systems submits your bill to your insurance company before looking to you for payment. Because we do not see patients before surgery, we rely upon the facility to provide us with the appropriate information. If the facility does not provide us with your health insurance information, you will receive a statement shortly after your surgery. Contact us immediately if you have health insurance coverage. Most insurance companies have limitations on the time frame within which a claim may be filed, it is critical that you check this information carefully at the outset to ensure that your insurance company does not reject your claim for untimely filing, leaving you responsible for the total bill. Anything you can do to expedite your insurance payment process will serve in your best interest.
You will receive a billing statement if we do not receive payment, if the claim is denied or if your insurance company is requesting more information.
Credit Policy
All charges are due and payable upon receipt of the initial billing. If you cannot pay the balance in full, please call our business office to arrange a mutually agreed upon payment plan.
You are responsible for payment of your account. If there is an overpayment by you, it will be refunded.
Finance Charges
A finance charge may be computed as a monthly period rate multiplied by the balance over 90 days. The balance over 90 days is determined by taking the portion of the balance owed at the end of the current billing cycle which is over 90 days old and subtracting any unpaid finance charges, payments and credits applied during the current billing cycle. No finance charges will accrue on amounts which are less than 90 days old.

IN CASE OF ERRORS OR QUESTIONS ABOUT YOUR BILL
If you think your bill is wrong, or if you need more information about a transaction on your bill, write us on a separate sheet at the address shown on your bill as soon as possible. We must hear from you no later than 60 days after we sent you the first bill on which the error or problem appeared. You can telephone us, but doing so will not preserve your rights. In your letter, give us the following information:
• Your name and account number
• The dollar amount of the suspected error
• Describe the error and explain, if you can, why you believe there is an error. If you need more informationd escribe the item you are unsure about.
You do not have to pay any amount in question while we are investigating, but you are still obligated to pay the parts of your bill that are not in question. While we investigate your questions, we cannot report you as delinquent or take any action to collection the amount in question.
